Lenalid 5 — Lenalidomide 5 mg
Lenalid 5 mg is the lowest strength capsule of the immunomodulatory drug Lenalidomide.
Manufacturer: Natco Pharma (India). Generic version of Revlimid.
Specific Use Case:
This 5 mg dosage is crucial for renal dose adjustments. It is designed for patients with compromised kidney function (Renal Impairment) or for those who experience severe adverse reactions (like neutropenia/thrombocytopenia) on higher doses.
✅ Benefit: Allows continuous therapy for fragile or elderly patients.
Indicated for the treatment of:
- Multiple Myeloma (MM): Often used for maintenance therapy post-transplant or for dose reduction due to toxicity.
- Myelodysplastic Syndromes (MDS): For patients with deletion 5q cytogenetic abnormality and renal issues.
- Mantle Cell Lymphoma.
Strength: 5 mg capsule.
Dosing for Renal Impairment:
- Moderate impairment (CrCl 30-60 mL/min): Often adjusted to 10 mg daily.
- Severe impairment (CrCl < 30 mL/min, not on dialysis): 15 mg every 48 hours (using combinations) or specifically titrated.
- End Stage Renal Disease (on Dialysis): 5 mg once daily. On dialysis days, administer the dose AFTER dialysis.
⚠️ Do NOT break or open capsules. Teratogenic hazard.
🚫 PREGNANCY WARNING (Black Box): Must not be used by pregnant women. Causes severe birth defects or fetal death.
Other Contraindications:
- Hypersensitivity to lenalidomide.
- Breastfeeding.
Even at 5 mg, significant risks exist:
- Venous Thromboembolism: Deep Vein Thrombosis (DVT) and Pulmonary Embolism (PE). Anticoagulant prophylaxis is required.
- Hematologic: Neutropenia and Thrombocytopenia (requires regular CBC monitoring).
- Constitutional: Fatigue, asthenia, constipation, and rash.
