Dosage and administration

Strength: 5 mg capsule. Dosing for Renal Impairment: Moderate impairment (CrCl 30-60 mL/min): Often adjusted to 10 mg daily. Severe impairment (CrCl < 30 mL/min, not on dialysis): 15 mg every 48 hours (using combinations) or specifically titrated. End Stage Renal Disease (on Dialysis): 5 mg once daily. On dialysis days, administer the dose AFTER dialysis. ⚠️ Do NOT break or open capsules. Teratogenic hazard.

Even at 5 mg, significant risks exist: Venous Thromboembolism: Deep Vein Thrombosis (DVT) and Pulmonary Embolism (PE). Anticoagulant prophylaxis is required. Hematologic: Neutropenia and Thrombocytopenia (requires regular CBC monitoring). Constitutional: Fatigue, asthenia, constipation, and rash.
