Avatrombopag (Doptelet): Treatment for Thrombocytopenia
Avatrombopag is an orally administered thrombopoietin receptor agonist (TPO-RA) designed to increase platelet counts in patients with thrombocytopenia. It works by mimicking the biological effects of thrombopoietin, the body's primary regulator of platelet production.
Avatrombopag (Doptelet) stimulates the megakaryocytes in the bone marrow to produce more platelets. It offers a convenient oral alternative to platelet transfusions for patients undergoing medical procedures.
It is widely used to manage low platelet counts caused by chronic liver disease or chronic immune thrombocytopenia (ITP).
Indications
- Chronic Liver Disease (CLD): Treatment of thrombocytopenia in adult patients with chronic liver disease who are scheduled to undergo a procedure (e.g., dental work, biopsy).
- Chronic Immune Thrombocytopenia (ITP): Treatment of thrombocytopenia in adult patients with chronic ITP who have had an insufficient response to a previous treatment.
Dosage and administration
Tablets must be taken with food.
- For Chronic Liver Disease (Pre-procedure):
- Duration: Administered for 5 consecutive days.
- Timing: Start dosing 10 to 13 days prior to the scheduled procedure.
- Dose: Depends on platelet count.
- Platelets < 40x10⁹/L: 60 mg (3 tablets) daily.
- Platelets 40–50x10⁹/L: 40 mg (2 tablets) daily.
- For Chronic ITP:
- Starting Dose: 20 mg (1 tablet) once daily.
- Maintenance: The doctor will adjust the dose to maintain platelet counts ≥ 50x10⁹/L to reduce bleeding risk.
- Thromboembolism: Known risk factors for thromboembolism (blood clots), as increasing platelet counts excessivey can be dangerous.
- Pregnancy and Breastfeeding (safety not established).
- Hypersensitivity to avatrombopag.
Side effects vary depending on the patient population.
- Common: Pyrexia (fever), abdominal pain, nausea, headache, and fatigue.
- Musculoskeletal: Arthralgia (joint pain).
- Edema: Peripheral edema (swelling of limbs).
- Serious Risks: Thrombotic/thromboembolic complications (blood clots).
