Mitomycin 40 mg is a high-strength vial of the antitumor antibiotic Mitomycin C.

Purpose of the 40 mg Vial:

Designed for high-dose chemotherapy regimens or hospital pharmacy compounding. It minimizes the number of vials needed for a single treatment session, reducing the risk of contamination and handling errors.

Key Use Case: Widely used in HIPEC (Hyperthermic Intraperitoneal Chemotherapy), where high concentrations are circulated in the abdominal cavity to treat peritoneal metastases.

Used in the treatment of:

  • Peritoneal Carcinomatosis: Via intraperitoneal administration (HIPEC) for gastric or colorectal cancer.
  • Bladder Cancer: High-dose intravesical therapy.
  • Systemic Cancer: Gastric, Pancreatic, and Anal cancers (IV route).

⚠️ DANGER: Potent Vesicant. Handle with extreme care.

Reconstitution of 40 mg Vial:

  • Usually reconstituted with 40 ml of Sterile Water or Saline to achieve a concentration of 1 mg/ml.
  • Ensure complete dissolution before administration.

Administration: Intravenous (IV), Intravesical, or Intraperitoneal. NEVER intramuscular or subcutaneous (causes severe gangrene/necrosis).

  • Coagulation disorders or increased bleeding tendency.
  • Severe bone marrow depression.
  • Pregnancy (Teratogenic).
  • Acute infection.

Delayed Toxicity Warning:

Myelosuppression (low blood counts) is delayed and cumulative. The nadir (lowest point) usually occurs 4-6 weeks after therapy. Dose adjustments are often required for subsequent cycles.

Other risks: Hemolytic Uremic Syndrome (HUS), interstitial pneumonia.
